Terms Of Enrollment

YOUR RESPONSIBILITIES:

1. To make a Last Will & Testament and get us a copy of it or any other documents or letters you want to be stored and be sent to recipient.
2. To provide accurate RECIPIENTS information.
3. To keep ALL information updated.
4. To pay your once a year $5 maintenance fee.

OUR RESPONSIBILITIES:

1. To check social security number and name with the (DMF) from the social security office.
2. To get verbal confirmation of your passing.
3. To contact RECIPIENTS using the information that you provided to assure and secure destination of your information.
4. To make all attempts send a Paper or Video copy of your information to all the RECIPIENTS that you have listed.

If your WILL RECIPIENTS information is not updated and or accurate, then we cannot be held responsible for undelivered documents, letters, or information. We will make all attempts to find and send all RECIPIENTS their copies, but no guarantees will be made due to lack of updated information.
